The MailmanProvisioner provisioned a new user when a petition was in the Pending Approval state. Later the petition was denied but the user remained provisioned in Mailman. Later when a different petition was approved the provisioner could not provision to Mailman because the user with the email address already existed.

It would be better if the MailmanProvisioner did not provision until the petition went into the Active state.
