In cases of status changes (eg someone leaving), how are people notified of this so that a request for access is denied/approved as it should be?

We need to understand more requirements for notifications and understand a reporting mechanism that has insight in to current state of users. We need to have a user interface that can expose this information easily.

Questions:
Would we want users to have access to this list? Are there any secret lists?
